TERMS AND CONDITIONS Members agree to follow Chosen Life Specialists, LLC rules and regulations. Violation of the rules and regulations may result in cancellation of your membership. Members will be responsible for payment in full upon revocation of membership. Inappropriate behavior from clients, guests or coaches will not be tolerated in any manner. Please notify the Chosen Life Specialists, LLC immediately if you have any concerns. Chosen Life Specialists, LLC reserves the right to refuse or discontinue service for any reason. Members agree that if they experience any physical pain or discomfort during the session, they will immediately inform the coach. Pricing, hours, and policies are subject to change and may vary per location. We reserve the right to change Chosen Life Specialists, LLC rules or pricing at any time with ten (10) days notice. As a member you receive a discount off certain services and programs during the term of your membership. Payment for services are due at the time of sale. All sales are final. Programs must be paid in two or less payments to receive the membership rate. For every six (6) months of membership you may bring in a friend to redeem one of your prepaid services. The membership may be extended to family at any time. Chosen Life Specialists, LLC is not responsible for lost or stolen property during sessions, at events and/or business matters/experiences. CANCELLATION POLICIES AND PROCEDURES Appointment Cancellation: Chosen Life Specialists, LLC requires 24-Hour cancellation notice. Cancellations within 24-Hours are charged a $30 cancellation fee. No shows by the client, family member or friend utilizing membership and/or other services will be charged 100% of the service fee.
